Guaranteed-issue rights are federal protections that require Medigap insurers to sell you a policy without medical underwriting — they can't deny coverage or charge more for pre-existing conditions. Guaranteed-issue applies during your initial 6-month Medigap Open Enrollment Period (turning 65 + enrolled in Part B), and during specific Special Enrollment Periods (losing employer coverage, Trial Right within 12 months of joining MA, etc.). Five states (NY, MA, CT, ME, VT) have year-round guaranteed-issue. Outside guaranteed-issue periods, most states allow medical underwriting.
