IRMAA stands for Income-Related Monthly Adjustment Amount. It's a surcharge added to your Part B and Part D premiums if your Modified Adjusted Gross Income (MAGI) exceeds federal thresholds. 2026 IRMAA is based on your 2024 tax return. For singles, surcharges begin at MAGI above $106,000; for couples filing jointly, above $212,000. Five income tiers exist — at the top tier (>$500K single / >$750K joint), the total Part B premium climbs to about $629/month (up from $185 base). About 8% of Medicare enrollees pay IRMAA. If your income dropped due to a life-changing event (retirement, divorce, death of spouse, work cessation), you can file Form SSA-44 to appeal.
