A copay (or copayment) is a fixed dollar amount you pay for a covered service at the time of service. Common amounts: $20-$45 office visit, $50-$300 specialist visit, $10-$50 generic prescription, $250-$500 emergency room. Copays may or may not count toward the deductible (depends on plan); they always count toward the out-of-pocket maximum. Plans with 'first-dollar coverage' have copays apply before the deductible is met; HDHP plans often require deductible to be met before any copays apply.
